Something at nursery pickup is bothering me and I don't know if I'm overreacting

My daughter is 2 and has been at her nursery for about five months. Generally happy, waves us off, no issues.

Twice in the last fortnight I've arrived for pickup and found her in the same nappy she was wearing at lunchtime (I know because I put a specific one in her bag that morning, the ones with the little stars, as she'd run out of the plain ones). The second time she had quite a sore red bottom by bedtime.

The daily sheet both times said she'd been changed at 2pm and 4pm.

Am I being paranoid? I don't want to be that parent but the sheet being wrong is what's really bugging me.

You're not being paranoid. I've worked in settings and the nappy itself is a minor thing, but the record not matching what actually happened is a big deal. Those records are how they evidence care. If they're filling them in from memory at the end of the day or ticking boxes, that's a practice problem.

Raise it calmly with the manager, not the room staff. Stick to facts: this nappy went in, this nappy came out, sheet says two changes.

This happened to us. Turned out they'd had two agency staff covering and one of them was writing the sheets for the whole room. Manager was mortified and sorted it immediately. Doesn't mean it's nothing, but it might be a staffing blip rather than something sinister.

I think pulling out straight away is a bit much before you've even asked. Give them a chance to explain and see how they respond. How they handle being challenged tells you loads.

Keep a little note on your phone of dates and what you saw. Not to build a case, just so you're not relying on memory if it happens again.

Spoke to the manager. She was really good about it, said they'd been short in the toddler room and one person was doing the sheets for everyone at the end of the day. They've changed it so whoever does the change logs it there and then, and she's doing spot checks. I'll keep an eye but I feel a lot better that she took it seriously.
